Clinical Residency and Fellowship Program Credentialing
The process for submission of the Annual Report and annual fee has
changed. Please read the instructions below carefully.
The 2009 Annual Report is due on or before January 31, 2010.
If the program was credentialed in 2009, please complete the Annual
Report for the portion of the year after the program was credentialed.
All programs are required to submit an Annual Report.
Use the link below to download the Annual Report in Microsoft
Word. Save the file onto your hard drive before and after completing the
report.
- Please type responses directly into this document. Additional pages
may be appended as needed to provide complete responses.
- If the Committee requested specific information during credentialing
or in response to the program’s 2008 Annual Report,
please append the requested items to this report.
- The residency/fellowship program director/coordinator and the
administrator of the umbrella organization sponsoring the
residency/fellowship program must sign the annual report (Electronic
signature is acceptable.).
- Completed forms should be mailed to Kendra Harrington, APTA,
Associate Director, Post Graduate Certification, P.O. Box 79054,
Baltimore, MD 21279-0054.
- You must include the Payment Form on last
page of packet.
- Please note: Annual Report & Fee must be submitted
together.

An invoice for the annual credentialing fees will be issued by the
Accounting Department. Please indicate that the check is for
residency/fellowship annual fees to assure that the Program payment is
documented appropriately. The fee is determined by the number of
residents/fellows in the program.
