The attached document is the Program Evaluation form utilized by the Clinical Residencyand Fellowship Program Credentialing Committee throughout the application review process. The tool includes all the credentialing requirments as indicated on the application. Programs can utilize this tool to self assess their residency and/or fellowship program.
