Held 3 times a year, these workshops are designed to prepare faculty and program administrators to develop a self-study report and host an on-site visit. For dates and registration information, click here.

Self-study Report Instructions (Word): Includes instructions, definitions, Evaluative Criteria and Evidence of Compliance Listings
Self-study Report Format (Word): Includes only Evaluative Criteria
Self-study Report Forms Packet (Word): All required forms, including the CV and scholarship forms

On-Site Visits (OSV)
PT OSV Forms: 3 forms are provided to the team at the beginning of the on-site visit. In addition to the 2 forms in the Items Provided On-site (Word) document, the General Information Form is also provided, revised if necessary to reflect changes since the Self-study Report was submitted. All 3 forms are provided to the team in hard copy and electronically.

A sample OSV Schedule is located in the Additional Information About the Accreditation Process for Candidates and Established Programs section of the Accreditation Handbook.

Substantive Change
Application for Approval of Substantive Change (AASC): please contact staff to obtain required forms. Rules are in the Part 9 of the Accreditation Handbook

Accreditation Handbook: Includes CAPTE Rules of Practice and Procedures, the Evaluative Criteria, Position Papers and information about accreditation fees.
Accreditation Updates: CAPTE newsletter published twice a year; provides information on the latest rule changes and CAPTE actions.
PT Education Fact Sheets: Demographic information collected on an annual basis about PT education programs. NOTE: Calculations are in progress and additional charts will be added when available.
