APTA published material is protected by copyright. All rights are
reserved. For information on how to obtain reprint permission, please
follow the instructions listed below. Approval for requests will be
issued by fax or US mail.
• OPTIMAL:
The Centers for Medicare and Medicaid Services has named APTA's OPTIMAL as one of the measurement instruments
clinicians are encouraged to use to document that a patient qualifies
for an automatic exception to the outpatient therapy cap. While APTA
intends to permit use of OPTIMAL at no charge to 1) clinicians within
their facilities for patient care, 2) bona fide education institutions
using material for in-classroom use, and 3) entities conducting
research, it is the copyrighted intellectual property of Physical
Therapy (PTJ) and so permission is required. Entities wishing to use
OPTIMAL in commercial products or services, including other
publications, also must apply for permission; products/services other
than publications potentially will be required to enter into a licensing
agreement with APTA for such use.
Students and other academic customers may receive permission to reprint
copyrighted material from Physical Therapy by contacting the Copyright
Clearance Center Inc, 222 Rosewood Dr, Danvers, MA 01923. All other
requests must be submitted in writing to APTA or via the online Reprint
Permission Form. Clinician requests must specify that OPTIMAL is to
be used for patient care within the facility at no charge. Other
requests must include title or type of publication/product in which
OPTIMAL will appear, estimated size and demographic of audience, or
print quantity of publication, and scope of permission. Written requests

audience or print quantity of publication, scope of permission, and, if
material is being adapted, the proposed revised text or graphic.
Note to Physical Therapy authors: If you want to reprint a figure
from one of your own articles in a new PTJ article, permission

• Reprint Permission Fees:
Except for bona fide educational institutions using material for
in-classroom use, APTA charges a fee for reprint permission. The fees
are dependent upon the amount of material being used, with a minimum fee
of $25 for members, $45 for nonmembers. Once we have received a
written request, we will determine the appropriate fees and will issue a
letter/invoice. Permission will not be granted until prepayment has been
received. APTA accepts credit cards (American Express, Visa, and
MasterCard), checks, and money orders as forms of payment. NOTE:
Certain uses of APTA content, such as in, but not limited to, commercial
software products, will require a licensing agreement rather than
copyright permission. APTA will notify you if we determine that your
request requires a licensing agreement.
• Permission Approval:
When APTA has received all required information and applicable fees,
permission approval will be issued via fax or mail. Permission will be
granted under the signature of APTA Director of Publications, and the
approval letter will state the following:
Permission to reprint or otherwise reproduce for non-exclusive use for
only the purpose listed in the attached request is granted with the
understanding that duplicated material carry a full citation: "Reprinted
from [author's last names/first initials, title of article, name of
publication, year, vol no., and page nos.], with permission of the
American Physical Therapy Association. This material is copyrighted, and
any further reproduction or distribution is prohibited."
Should APTA, in its sole discretion, determine that your use of our
material is contrary to the original intent as we understood it in your
request for permission, or is no longer appropriate, we reserve the
right to revoke this permission of your use of APTA material.
As a courtesy (if applicable), please also contact the senior author for
permission.
Depending on the request, APTA may also require additional citations
and/or a link to APTA's Web site. If additional citations and/or a link
are required, the permission letter will clearly indicate this.
