Clinical Residency and Fellowship Program Credentialing Application Resource Manual

The Clinical Residency and Fellowship Credentialing Application Resource Manual was created by the Clinical Residency and Fellowship Program Credentialing Committee with cooperation and assistance from APTA credentialed residency and fellowship program directors who provided actual examples of application requirements ("Evidence"). The intent is to assist developing programs by providing examples of how other programs have met the application requirements. The examples are not meant to be prescriptive and any reproduction requires approval from the author.
