    #Guest Rate: Guests must register with conference attendee. Please note that children under age 16 are not permitted in the Exhibit Hall. Guests have access to the Opening Reception, ABPTS Awards Ceremony, and the CSM Exhibit Hall.

    Badge & Conference Materials

    Those who reside in the US and register for CSM by January 5, 2015, will receive their meeting materials via mail in advance. All registrants living outside the US, or who register for CSM after January 5, 2015, will be required to pick up their meeting materials in Indianapolis during the posted registration hours.

    Cancellations and Refunds

    This cancellation policy applies to all elements of the conference. There is no separate fee for cancelling participation in preconference courses.

    • A $60 fee will be assessed to cancel between the time of registration and January 23, 2015.
    • There will be a $20 fee to switch registrations (substitute one attendee for another).
    • No refunds will be given after January 23, 2015.

    Cancellation for Confirmation E-mail

    Cancellation requests for preconference and/or full conference received by January 23, 2015 will receive a full refund minus a $60 administration fee. There will be no refunds for no-shows or cancellations after January 23, 2015. Cancellation requests must be submitted in writing to the APTA Registration Center, 11208 Waples Mill Rd., Ste 112, Fairfax, VA 22030 or e-mailed to aptaregistration@jspargo.com.
