The Clinical Residency and Fellowship Credentialing Application Resource Manual was created by the American Board of Physical Therapy Residency and Fellowship Education with cooperation and assistance from APTA credentialed residency and fellowship program directors who provided actual examples of application requirements ("Evidence"). The intent is to assist developing programs by providing examples of how other programs have met the application requirements.
The examples are not meant to be prescriptive and any reproduction requires approval from the author. This document has recently been updated to reflect the January 2010 updates made to the credentialing application, however additional examples are currently being gathered.
*PLEASE NOTE: The application resource manual does not align with the 2013 credentialing application. Please make sure you follow the 2013 application for number the evidence and not the number located within this resource manual. An updated resource manual will be posted shortly.
