    The Triple Aim is emphasized by all Twin Cities care systems with a slight variation - measurable quality, exceptional patient experience, lower per capita cost. Therapy Partners - a management services organization for independent physical therapy practices - has adopted "delivering triple aim value" as an aspect of our mission. We proved that value in 2010-11 with a value based reimbursement model with HealthPartners (HP) health plan. Therapy Partners' practices used FOTO outcomes in all clinics and worked as a team with HP in this very successful model showing health plan and providers can work collaboratively to provide meurable value to patients, employers, therapists, and health plan.
