  • Sample Privacy Notices Now Available for Health Care Providers

    If you need help creating a Notice of Privacy Practice to comply with the HIPAA privacy rules, the federal government has developed model notices that you can customize, reproduce, and distribute either in print or electronically to patients.

    The Health Portability and Accountability Act (HIPAA) give patients a right to be informed of the privacy practices of their health care providers and health plans and of their privacy rights regarding their personal health information. HIPAA requires providers and plans to notify patients of these rights. By using a model notice, you will know you are providing the information in compliance with the regulations and that the notice reflects recent regulatory changes to the HIPAA privacy and security rules.

    You can download the model notice as a booklet, as a multipage document, as a single page presentation, or as text only.

    APTA includes this and many other resources on its HIPAA webpage.

    • Thank you for this help!! I just wish it had come sooner--I mean only 2 working days until mandatory compliance?! Most of us have already spent lots of time and energy on this.

    • This has been a concern and worry and I have spent hours trying to get a complete HIPPA form in place. I appreciate and will use this but 2 days before mandated?? Good grief.

    • APTA volunteers and staff: thanks again for making us aware of changes and new regs. So very grateful to have sample forms etc. Great timesaver but more importantly, gives me assurance I will get it right and that I do not have to recreate or start from scratch -- thanks again.

    • Starting up a brand new practice is a scary thing. Especially if you grew up in poverty. Having access to forms like these is a real blessing for those of us just entering the field, saving us lots of time. Having required documents that are correctly put together according to the law keeps us from having to be lawyers in order to practice our medicine. So to those of you who put so much effort and time into creating these and making them available to us, thank you. Blessings to you. Peace to you and yours.
