Report Your Advocacy Activity

Completing the following forms allows APTA to track the actions being taken by members, helping APTA staff follow up with members of Congress and their staff.

• Grassroots Activity Report Form

  Fill out this form when you send a personal letter, host a practice visit, attend a town hall meeting, or otherwise meet with one of your legislators.

• Legislative Action Alert Response Form

  Fill out this form only if you used APTA's Legislative Action Center and wish to record other activities, such as personal letters or phone calls, or any other actions taken.

• Student Advocacy Challenge Report Form

  For participants of the Student Advocacy Challenge only.
