California State University, Sacramento Graduate Admissions Application - Applicants need to apply for admission and be accepted to California State University, Sacramento by 11/30/2009 in order to attend the physical therapy Program. This application can be found beginning 10/01/2009 and submitted online at www.csus.edu/gradstudies/.      

Department of Physical Therapy Supplemental Application - Applicants must also complete and submit the Department of Physical Therapy Supplemental Application by 1/15/2010 for their physical therapy application to be considered. This application can be found beginning 7/15/2009 and submitted online at www.hhs.csus.edu/pt/.  If you are unable to submit this application online, it can be downloaded and mailed to:

Applicants must complete both applications in order to be considered for admission.  Please be aware that application to the University Graduate Admissions is a separate process than application to the Physical Therapy department.

The California State University, Sacramento Graduate School Application:  due by November 30, 2009.  This graduate school application has a $55.00 non-refundable application fee.

The Department of Physical Therapy Supplemental Application: due by January 15, 2010. The Department of Physical Therapy has no supplemental fee.  This application, in part, addresses "Additional Background Criteria" considered in the ranking of students for admission to our program.  The three areas considered include:    Bilingual Language Competency, Economic, Environmental, and Educational Background, and Advanced Academic Courses.

Additional information about program’s reference requirements:Three letters of recommendation on the provided forms need to be completed. Two of the forms need to be completed by U.S. or Canadian licensed physical therapists who have worked with the applicant in a physical therapy clinic. The supervision needs to have occurred when the applicant was either a volunteer or an employee at the clinic. The third letter of recommendation must be provided by a college level professor (either major or non-major) who has been an instructor of the applicant.

Additional information about program’s PT hours requirement

Applicants need to have accumulated and documented a minimum of 100 hours of volunteer or paid experience. Hours need to have occurred in at least two different U.S. or Canadian physical therapy settings (inpatient, outpatient, home, SNF, pediatric, acute hospital, etc.) under the direct supervision of a licensed physical therapist. Minimum acceptable number of hours at a single setting for the setting to be applied to this requirement is 25 hours.

GPA computation uses most recent grade of a course repeated one time; uses average of grades earned for courses repeated more than once. 

Courses that need to have been completed within the last 10 years include anatomy, physiology, kinesiology, exercise physiology, pathophysiology, and pharmacology.

Advanced anatomy can be substituted for the basic anatomy prerequisite if it is to your advantage to do so. If advance anatomy is used for the basic anatomy prerequisite, no additional points can be earned on the supplemental application for taking advanced anatomy.

Additional information about program’s policy on foreign coursework 

Foreign applicants need to have at least 8 units of prerequisite coursework completed prior to 10/15/2009 at a US accredited institution.  Applicants who do not possess a bachelor's degree from a post secondary institution where English is the principal language must demonstrate competence in English by scoring a minimum of 550 (213 if computer-based) on the Test of English as a Foreign Language (TOEFL). Foreign applicants must complete their observation hours in US physical therapy clinics.

PROGRAM DESCRIPTION

MISSION: The mission of the California State University, Sacramento, Department of Physical Therapy is to educate competent physical therapist generalist practitioners who demonstrate ethical, responsible, professional behavior, use evidence derived from research to inform clinical practice, are sensitive to cultural and psycho-social differences, utilize critical thinking and problem solving, practice life long learning, and provide services that contribute to the optimal health and function of the residents of the communities they serve. 

CURRICULUM: The curriculum of two and half years, including summers, encompasses didactic instruction, laboratory experience, and clinical education. The clinical component consists of thirty weeks of full-time, off-campus clinical internships leading to a Certificate of Clinical Competence in Physical Therapy.
