Eligibility: Applicants who received their education outside of the United States (US) at a physical therapy program not accredited by the Commission on Accreditation for Physical Therapy Education (CAPTE) can still apply for membership if they now reside in the United States and submit a credentials evaluation or a license from a US jurisdiction that requires a credentials evaluation prior to licensure. If you are a foreign-educated PT or PTA living outside of the US, you may be eligible to become affiliated with APTA via the International Partners subscription.
Application: You must submit copy of your physical therapy license or credentials evaluation in addition to filling out the online application, or along with your faxed or mailed downloadable application form. Forms can be faxed to 703/706-8536 ("Attention: Member Services"), or mailed to APTA, 1111 North Fairfax St., Alexandria, VA 22314-1488. Important: Do NOT fax your application containing credit card information. You can fax your application and forms, but then call Member Services at the number below to submit your payment information.
