What it measures:
Diet and physical activity
Target Population:
Adults
Clinical Insights: Health care providers can use this quick and easy tool to assess and discuss with patients, in any order, their body mass index, physical activity, and diet/nutrition. The tool includes recommendations, based on patient response, to facilitate health promotion and disease prevention. Potential recommendations include provision of patient education and resources, joint goal-setting and referral to a dietician when indicated.
