The observation log is a valuable tool for physical therapy students to document their hands-on experiences and reflections in various clinical settings. Most physical therapy (PT) programs require applicants to complete observation or "shadowing" hours in one or more physical therapy settings. These experiences help applicants:
- Understand the roles and responsibilities of physical therapists
- Observe patient care, interventions, and documentation
- Gain exposure to diverse patient populations
- Confirm their interest in the profession
Although this observation log cannot be used to verify your hours in the Physical Therapy Centralized Application System (PTCAS), we encourage you to use it to track your hours and note important contact details for hours verification if necessary.
How to Use the Observation Log
Before You Begin Observing
Be sure to review and research any program of interest’s recommended and required hours on the PTCAS Directory. Utilize the Program tab in the observation log to keep track of your programs and their requirements.
Completing the Observation Log
Ensure you gather complete information, including details about supervising PTs or PTAs information, to ensure accuracy when completing your application in PTCAS. Use the notes column (column AD) to track specific populations or modalities observed during your experience. You can also use this section to indicate whether a supervising PT or PTA is open to writing a letter of recommendation or provide additional mentorship.
Verifying the Observation Log
Depending on program requirements, you may need to have PT or PTA verify your hours either electronically through the PTCAS system or by uploading a document with their written signature. This observation log cannot be used as a verification for the PTCAS application if a program requires it.
