Eligibility Requirements

Nominees must be an individual other than a member in any other membership category.

Criteria for Selection

Per APTA’s bylaws, nominees must be:

  • An individual, other than a member in any other membership category, who has rendered outstanding service to the association or made notable contributions to the health of humanity.

Submission Requirements

Nominations must be submitted by one of the following:

  • Component president
  • Component leader
  • Component executive

The nominator must submit a nomination statement (nomination statements are 300-words) to begin the process.

The nominator is responsible for the following:

  • Submitting nominee’s current curriculum vitae or resume
  • Completing an online application (application questions are 500-words)
  • Proposed language for presentation of the nomination to APTA's House of Delegates in the form of a resolution with a preamble (see nomination form for reference) 
  • Supporting materials as deemed appropriate

The nominations call for the Honors & Awards Program is open from Sept. 1 to Dec. 1. Nominations must be submitted through APTA Achieve by Dec. 1; late submissions will not be considered. Selected recipients and their nominators will be notified in May.
