By completing this sign-up form, you will be identified as a member of the network (no fees required). You should select one or two APTA members who are leaders in a major health system to become the primary and secondary contacts to represent the system.

You also will be subscribed to receive email updates from the network's online community at the Hub (you may adjust your email alerts at any time). You can access the online community immediately, but please allow three business days to complete your onboarding, and note that the volume and frequency of email updates from the council will vary.
