Eligibility: Entrants must be members of the American Physical Therapy Association as of date of entry. Employees, officers, and directors (and their immediate family members and those living in the same household) of Sponsor, its affiliates, parents, subsidiaries, vendors, consultants, contractors, advertising and promotion agencies, public relations, promotion, fulfillment and marketing agencies, and web site providers/web masters are not eligible to enter or win. Sponsor reserves the right to verify the eligibility of winners.

By entering, Entrants agree to release, discharge, indemnify, and hold harmless Sponsor, its affiliates, parents, subsidiaries, advertising and promotion agencies, and their respective officers, directors, employees, representatives, and agents, from any injuries, losses, liability, or damages of any kind resulting in whole or in part from acceptance, possession, or use of any prize, or participation in this contest.

Selection of the Best Videos: Videos that best fit contest criteria will be selected by random drawing on or about July 12, 2021.  APTA is giving away APTA Centennial Journals to the first 50 members who submit by the deadline videos that meet the contest requirements. Return of a journal or recipient notification as undeliverable may result in disqualification and the gift may be awarded for an alternate video submission. Gift recipients will be notified by email and/or telephone within five (5) business days of the drawing and will be provided instructions on how to redeem the gift.  If winner does not claim his/her journal within 15 days of notification, it may be awarded to an alternate winner via a second chance random drawing. Gift recipients may be required to sign an Affidavit of Eligibility and a Liability and Publicity Release form (collectively “Release forms”) and return the Release forms within 15 days of winner notification.  If the Release forms are not returned by the deadline, winner may forfeit gift. Forfeited gift will be awarded in a second chance random drawing.

Failure to comply with any of these Official Rules will result in forfeiture of the prize. The prize will be awarded in coordination with each individual winner and the Sponsor.  Except where prohibited by law or regulation, a winner's acceptance of a prize will constitute permission for Sponsor to use winner's name, likeness, and statements for any purpose in reference to the contest and this promotion without additional compensation, consideration or consent. If by reason of a print or other error, more prizes are claimed than the number set forth in these official rules, all persons making purportedly valid claims will be included in a random drawing to award the advertised number of prizes available in the prize category in question. No more than advertised number of prizes will be awarded.  Sponsor reserves the right to cancel this promotion at any time without notice or obligation.

Any and all applicable local, state, and federal taxes are solely the responsibility of the winner(s). Prizes cannot be redeemed for cash, transferred, assigned or substituted, except that Sponsor reserves the right to substitute a prize with a prize of equal or greater value at its sole discretion.

General Conditions: Sponsor, its affiliate companies, parents, subsidiaries, officers, directors, and agents are not responsible for lost, late, misdirected, unintelligible, illegible, stolen, returned, or undelivered, entries, mail or e-mail, or for lost, interrupted or unavailable network, server, Internet Service Provider (ISP), website, or other connections, availability, accessibility or traffic congestion, miscommunications, failed computer network, telephone or cable hardware or software or lines, or technical failure, jumbled, scrambled, delayed, or misdirected transmissions, computer hardware or software malfunctions, failures or difficulties, or other errors of any kind whether human, mechanical, electronic or network. Persons who tamper with or abuse any aspect of the contest, as solely determined by the Sponsor, will be disqualified, and Sponsor reserves the right to terminate such entrant’s eligibility. Sponsor is not responsible for any incorrect or inaccurate information whether caused by website users or by any of the programming or equipment associated with or used in this contest, and assumes no responsibility for any errors, omission, deletion, interruption or delay in operation or transmission or communication line failure, theft or destruction or unauthorized website access.

Sponsor is not responsible for injury or damage to participants or any other persons, computer or property related to or resulting from participating in this contest. Should any portion of the contest be, in Sponsor’s sole opinion, compromised by virus, worms, bugs, non-authorized human intervention or other causes which, in the sole opinion of the Sponsor, corrupt or impair the administration, security, fairness or proper play of the contest, or submission of entries, Sponsor reserves the right at its sole discretion to suspend, modify or terminate the contest.

BY PARTICIPATING IN THIS CONTEST, ENTRANTS AGREE THAT APTA, ITS AFFILIATES, PARENTS, SUBSIDIARIES, AGENTS, AND THEIR RESPECTIVE OFFICERS, DIRECTORS, EMPLOYEES, REPRESENTATIVES, VENDORS AND AGENTS WILL HAVE NO LIABILITY WHATSOEVER FOR, AND SHALL BE HELD HARMLESS BY ENTRANTS AGAINST, ANY LIABILITY, FOR ANY INJURIES, LOSSES OR DAMAGES OF ANY KIND TO PERSONS, INCLUDING DEATH, OR PROPERTY RESULTING IN WHOLE OR IN PART, DIRECTLY OR INDIRECTLY, FROM ACCEPTANCE, POSSESSION, MISUSE OR USE OF THE PRIZE OR PARTICIPATION IN THIS CONTEST OR IN ANY CONTEST RELATED ACTIVITY.
