PTs working with children who have disabilities can help them accomplish amazing things. For youth who will continue to need physical therapy as adults, one of those things should be helping them transition to a PT who works with adults.

The Individuals with Disabilities Education Act requires that a formal transition be started by age 16 for school-based services, and many states begin at age 14.
